Navratri & Karva Chauth Fasting with Diabetes, PCOS or Thyroid
You can keep your vrat and keep your health. A clinical dietician's guide to what to eat, what to skip, how to handle your medicines, and when to break the fast safely.

Short answer: Most people with well-controlled type 2 diabetes, PCOS or hypothyroidism can fast during Navratri if they plan it. That means eating small meals every 3–4 hours instead of one big meal, building each plate around protein (paneer, curd, nuts) rather than sabudana and aloo, never changing medicine on your own, and checking blood sugar if you have diabetes. A nirjala (no water) fast like Karva Chauth carries more risk, especially on insulin or certain diabetes tablets, so speak to your doctor first.

Talk to your doctor before fasting if any of these apply
- You take insulin, or diabetes tablets that can drop sugar too low (such as glimepiride or gliclazide). The dose or timing usually needs changing for a fast.
- Your sugar has been uncontrolled recently, or you have had a low-sugar episode (hypo) in the last few months.
- You have kidney disease, heart disease, or diabetes-related complications.
- You are pregnant or breastfeeding. A nirjala fast is generally not advised.
- You were recently diagnosed with thyroid disease and your dose is still being adjusted.
Faith often allows exceptions for health. Many families and priests accept a phalahari fast (fruits, milk, vrat flours) instead of a water-only fast for people with medical conditions.

Vrat food is often more sugary and starchy than everyday food. Sabudana, aloo, fried pakode and kheer all push blood sugar up fast. Here is how the common vrat foods compare for diabetes, PCOS and thyroid.

What changes for diabetes, PCOS and thyroid
Fasting with type 2 diabetes
- Eat small, eat often. Five or six small meals keep sugar steadier than one heavy vrat thali in the evening.
- Never stop or change medicine on your own. Insulin and some tablets can cause dangerously low sugar during a fast. Ask your doctor how to adjust the dose or timing before Navratri starts.
- Check your sugar at least twice a day while fasting (morning and 2 hours after your main meal), and more often on Karva Chauth.
- Pair every carb with protein. A kuttu roti with paneer sabzi behaves very differently from a kuttu roti on its own.
- Drink enough water through the day on a phalahari fast. Dehydration raises sugar readings too.

Fasting with PCOS
- Most women with PCOS have some insulin resistance, so the same rule applies: protein first, small carb portions, very little sugar.
- Don't turn the vrat into a crash diet. Eating almost nothing for nine days often ends in a binge on Dussehra and weight rebounding by Diwali.
- Protein at every meal: paneer, curd, milk, nuts, seeds. It keeps hunger and cravings calmer.
- Keep moving. Garba and dandiya count as exercise. So does a 30-minute evening walk.
- Protect sleep. Late-night pujas and celebrations add up; poor sleep makes cravings and cycles worse.

Fasting with hypothyroidism
- Take your thyroid tablet at the same time every day, on an empty stomach with water, 30–60 minutes before food. Fasting does not change this.
- Keep milk, curd, paneer and calcium tablets about 4 hours away from your thyroid tablet, as calcium reduces how well the tablet is absorbed. Iron supplements need the same gap.
- On Karva Chauth, ask your doctor how to handle the tablet. Don't skip doses for several days in a row.
- Expect lower energy if you already feel tired. Long gaps without food make it worse, so keep small meals regular.

A sugar-friendly 9-day Navratri plan: Days 1–3
A phalahari plan built for steady sugar and steady energy. Portions here are general; in consultation we adjust them to your weight, reports and medicines. Follow your family's vrat rules on which vegetables and spices are allowed.

- Warm water + 1 tbsp pumpkin seeds
- Breakfast
- Rajgira porridge in milk, no sugar, cinnamon and chopped nuts
- Mid-morning
- 1 pear or small bowl papaya
- Lunch
- 2 small singhara rotis, lauki-tomato sabzi with paneer cubes, chaas
- Evening
- Small handful roasted peanuts + chaas
- Dinner
- Air-fried paneer tikka, sautéed vegetables, bowl of curd
- Early morning
- Warm water + 5 soaked almonds
- Breakfast
- Kuttu chilla stuffed with paneer, mint curd dip
- Mid-morning
- 1 orange
- Lunch
- Samak rice vegetable pulao (small bowl), cucumber raita
- Evening
- Makhana chaat with tomato, cucumber and lemon
- Dinner
- Lauki-paneer soup, 1 rajgira roti

Karva Chauth with diabetes, PCOS or thyroid: sargi to moonrise
A nirjala fast means no food and no water from sunrise to moonrise, often 13–14 hours. Here is how to prepare your body for it.

A week before: check with your doctor
Especially if you take insulin or sugar-lowering tablets. Ask what to do with each medicine on the day, and whether a phalahari version (water, milk, fruit) is safer for you.

Sargi (before sunrise): build a slow-release meal
- 1–2 glasses of water, sipped steadily (not gulped at the last minute)
- Protein: paneer or besan-paneer chilla, or a multigrain paneer paratha
- A bowl of curd and a small handful of nuts
- One fibre-rich fruit such as guava or apple
- Keep feni, mathri and sweets to a token portion. Sugar now means a crash and hunger by noon.
- Thyroid tablet: take it as your doctor advised for the day

During the day: conserve energy
Stay in a cool place, skip heavy housework and long hours in the kitchen, and rest after the afternoon katha. If you have diabetes, check your sugar around midday and again before evening puja.

Moonrise: break the fast gently
- Start with plain water, sipped slowly
- Then 1 date or a few pieces of fruit, not a plate of mithai
- Wait 15–20 minutes, then a normal dinner: dal, sabzi, 2 rotis, salad, curd
- Keep fried food and sweets small. A heavy feast after 14 hours sends sugar soaring.
Safety
When to break the fast immediately
No vrat is worth a hospital visit. If you have diabetes, break your fast straight away if any of these happen:
Signs of low sugar
- Shaking, sweating, racing heartbeat
- Sudden hunger, dizziness or headache
- Confusion, blurred vision, slurred speech
- Glucometer reading below 70 mg/dL
Signs of high sugar or dehydration
- Extreme thirst, dry mouth, passing very little urine
- Unusual tiredness or weakness
- Nausea or vomiting
- Glucometer reading above 300 mg/dL
For low sugar, use the "rule of 15"
If the person is confused, can't swallow, or doesn't improve, get medical help immediately.

Festive fasting questions we hear every year
Can a person with diabetes keep the Navratri fast?
Often yes, if sugar is well controlled and the fast is planned: small meals every 3–4 hours, protein with every meal, regular sugar checks, and medicine changes only under your doctor's guidance. People on insulin or with recent low-sugar episodes should speak to their doctor before fasting.
Is sabudana okay for diabetes or PCOS?
Sabudana is almost pure starch with very little protein or fibre, so it raises blood sugar quickly. It's best kept to an occasional small portion, mixed with peanuts and eaten with curd. Kuttu, singhara, samak and rajgira, paired with paneer or curd, are better everyday choices.
Can I do a nirjala Karva Chauth fast if I have diabetes?
A water-free fast carries a real risk of low sugar and dehydration, especially on insulin or tablets like glimepiride or gliclazide. Discuss it with your doctor a week ahead. Many women choose a phalahari version with water and fruit instead. If you do fast, check your sugar during the day and break the fast if it drops below 70 or goes above 300 mg/dL.
When should I take my thyroid tablet during a fast?
Keep the usual time: on an empty stomach with water, 30–60 minutes before food, and about 4 hours away from milk, curd, paneer, calcium or iron. For a nirjala fast like Karva Chauth, ask your doctor how to handle that day's dose.
Does Navratri fasting help with PCOS weight loss?
It can, if the vrat is built on protein, vegetables and small portions of vrat flours. A fast built on sabudana, aloo and fried food often causes weight gain instead. Starving for nine days and then feasting on Dussehra usually undoes the benefit.
Is kuttu atta good for diabetes?
Kuttu (buckwheat) is a better choice than sabudana or aloo because it has more fibre and protein, but it is still a carbohydrate. Keep to 1–2 small rotis, mix in grated lauki, and avoid kuttu puris and pakode.
